You signed up for the Crabby Diamond on January 3rd at 1:00pm. Chesapeake sent over this waiver form for you to fill out before you arrive. If you happen to be going to the Funspiel later in January and already filled out the waiver for that event, then don’t worry about this one. It’s the same waiver.
